On this the 22nd day of October, 9984, the Commissioners
Court of Brazos County, Texas, convened in Regular Session at the
October term of the Court, at the Courthouse, in the City of Bryan,
County of Brazos, State of Texas, with the following members
present, to-wit
R J Holmgreen, County Judge
Bill Cooley, County Commissioner, Precinct No 1
Walter Wilcox, County Commissioner, Precinct No 2
Billy Beard, County Commissioner, Precinct No 3
Milton Turner, County Commissioner, Precinct No 4
Among other proceedings had, the following proposed order was
submitted
"WHEREAS, The Honorable Carolyn Ruffino, Judge of the County
Court at Law of Brazos County, Texas, having made known to the
Commissioners Court of Brazos County, Texas, that from October 27,
1984 through November 18, 1984, she will be absent from the bench
by reason of attending the Juvenile Justice Conference with a
delegation of Judges, probation officers and others working with
programs involving youth in trouble for the countries of the U S ,
Japan, China and Taiwan
"WHEREAS, she has requested the appointment of I Bradley
Smith to sit as Special Judge of the County Court at Law of Brazos
County, Texas on the above-mentioned dates in all matters that are,
or may be, docketed on the Court's dockets for those days, and to
have and exercise all powers of a Special Judge in relation to the
matters involved, it is, therefore,
ORDERED, ADJUDGED AND DECREED, pursuant to authority of
Article 1970-359 and 1933a, Vernon's Civil Statutes and Article
30 03, Section 2, Code of Criminal Procedure, that J Bradley Smith,
a former Judge of the County Court at Law of Brazos County be, and
he is, appointed as Special Judge of the County Court at Law of
Brazos County, Texas to sit in all matters that are, or may be,
docketed on any of the Court's dockets on the above-mentioned days
and shall have and exercise all powers of such Special Judge in

relation to the matters involved

WHERIAS, the Court having con%idered the proposed order and
deeming that it should be approved, it is, therefore,
ORDERED, ADJUDGED AND DECREED by the Commissioners Court of

Formal notice is hereby given that Clajon Gas Com aap., , Applicant,
proposes to place a 611 within t elright-of-way
of Allen Farm Road in Brazos County, Texas as follows
Road crossing only
_ Installation parlleling the roadway a distance of feet

The location or description of the proposed installation is more fully
shown by two copies of the drawings attached to this notice
I understand and agree that
1 The Commissioner of the Precinct iii which the right-of-way lies must
be notified prior to the beginning of construction in order that
the Commissioner might be on hand to designate the actual location
of the installation
z 2 That all damage to the roadways and rights-of-way will be repaired
to their original condition to the satisfaction of the Commissioner
in which the roadway or right-of-way lies
r
3 That BRAZOS COUNTY reserves the right to require Applicant to re-
locate or lower any such line at no cost to BRAZOS COUNTY, should
► same become necessary due to widening or lowering, or other alter-
ation of the roadway or right-of-way
4 That BRAZOS COUNTY is in no way responsible for any damage that might.
occur to any existing utility lines in the right-of-way

a 5 That the line will be constructed and maintained on the county right -a'
of-way in accordance with governing laws
6e That the line or lines will be constructed no less than forty-eight
inches (4811) lower than the lowest part of the drainage or bar ditch
and the drainage is to be considered at least two feet (2') below

7. That all roads be bored to forty feet (40') on either side of the
centerline of the right-of-way or cased for such distance iiJ11

8. That all sites will be barracaded during construction period

9 That the normal charge established by the Commissioners' Court is
$500 per crossing and/or $25 per rod when paralleling the roadway

Formal notice is hereby given that FERGUSON CROSSING PIPE LINE COMPANY

Applicant, proposes to place a 41'-Gas Line within the right-
of-way of yarisco Lane in Brazos County, Texas as follows
The location or description of the proposed installation is more fully shown
by Three (3) copies of the drawings attached to this notice -
I understand and agree that.
1 The Commissioner of the Precinct in which the right-of-way lies
must be notifl2d prior to the beginning of construction in order
that the Commissioner might be on hand to designate the actual ;
location of the installation v
2 That all damage to the roadways and rights-of-way will be repaired
to their original condition to the satisfaction of the Commissioner
in which the roadway or right-of-way lies

BRAZOS COUNTY GROUP MEDICAL PROGRAM
Policies and Procedures For Filing Groun Medical InWrance Claim Forms

1. Brazos County Group Medical Program hospitilization and dental claim forms
are to be obtained from the Personnel Department
2, An employee is to complete the Participant Employee`s Statement portion of
the claim form, provide the Explanation of Benefits from Blue Cross-Blue Shield
of Texas (if available) showing that you havr satisfied) your deductible for the

pei iod beginning 01/01/84 - 12/11/84 foi proof of deductible credit with Ora7r m
County Group Medical Progrun, include original receipts and return all paper-
work to the Personnel Department
Note An employee will need to complete only one initial claim form per
calendar year for verification of coverage After the first verification of
coverage claim form is submitted, a transmittal form will be used by the
Personnel Department to file each claim Receipts and bills are to be forwarded
to the Personnel Department for processing

3. The Personnel Department will make copies of the claims and receipts j
submitted to Employee Benefits Management, Inc (located in Dallas) for
Payment The Personnel Department will maintain an active file system for
the pending claims and a permanent file system for paid claims

4 once the claim has been processed by thployee Benefits Managemnt, Inc
the checks will be -forwarded to the Personnel Department for distribution.
